Support coordination
Support coordination for Frankston and the Peninsula
Support coordination is NDIS funding for help to understand your plan, connect with providers and get the most from your budget. Parcare Lifestyle provides NDIS support coordination in Frankston and across the Mornington Peninsula. Our Victorian office support is based in Carrum Downs, and our support workers live in Frankston South, Carrum Downs, Skye and Blairgowrie.
What a support coordinator does
- Explains your plan and what each budget can pay for
- Finds providers who will actually staff your supports, including on the southern Peninsula
- Sets up service agreements and keeps an eye on your budget
- Helps when something goes wrong, from a provider pulling out to a crisis at home
- Pulls together the evidence for plan reassessments and home and living requests
- Works with Frankston Hospital and Rosebud Hospital discharge teams when you are coming home
Choice stays with you
Using Parcare for support coordination never means you have to use Parcare for your other supports. Your coordinator puts every option in front of you, and you choose. Where we do deliver another support, we tell you upfront and manage it in line with the NDIS Code of Conduct.

Do I have support coordination in my plan?
Check the Capacity Building budget for Support Coordination. If it is not there and you need it, your LAC or the NDIA can consider it at your next plan reassessment.
Can I change support coordinator?
Yes. Give your current coordinator the notice in your service agreement and we take over, picking up where they left off.
How do I get started?
Send a referral or call us. A participant, family member, support coordinator or hospital discharge planner can refer. We call back, talk through your goals and plan, and match you with a worker who lives close by.
