Collection of Information Statement

Why we ask for personal information, and what we do with it.

Parcare Support Service Pty Ltd trading as Parcare Lifestyle collects, holds and uses personal information under our privacy policy and the Privacy Act 1988, including the Australian Privacy Principles. This statement is the collection notice for people who contact us, apply for supports, or become a participant.

What we collect

Depending on why you contact us, we may collect your name, contact details, date of birth, NDIS number, disability and support needs, health information, living arrangement, nominated representatives, and information needed to quote, roster or invoice. Website forms collect the fields you type (name, email, phone, message).

Why we collect it

We collect information to respond to an enquiry or referral, assess whether we can deliver supports, enter a service agreement, deliver and record NDIS supports, meet Practice Standards, worker-screening and incident duties, and meet other legal duties. Except where a law requires or authorises collection, we ask only for what we need for that purpose.

Who we collect it from

Where it is reasonable, we collect information from you. We may also collect it from a nominee, guardian, family member, support coordinator, another provider, or a government agency, where the law allows or you have authorised it.

Who we may disclose it to

We may disclose information to people acting for you, treating practitioners you have authorised, other NDIS providers in your team, the NDIA, the NDIS Commission, worker-screening agencies, auditors, insurers, and our contracted service providers (including VisualCare, Microsoft 365, Xero, Twilio, WordPress, and rostering and notes software we operate). We may disclose information without consent if a law requires or authorises it.

If you do not provide the information

If you do not give us information we reasonably need, we may be unable to process a referral, enter an agreement, or deliver the support. Optional items (photos for social media, marketing) can be refused without ending supports.

Access, correction and complaints

You can ask to see or correct information, withdraw optional consent, or complain. Write to the Director / Privacy Officer at privacy@parcaresupports.com or use the complaints page. You can complain to the OAIC at any time (1300 363 992). Full detail is in the full privacy policy.

Participants also sign a Privacy Consent Form when they start services. That form is the detailed consent. This page does not replace it.
